What Causes Cloudiness And Flakes Like Discharge During Urination?

This morning at work I went to urinate. At the end of urinating I noticed some cloudy, somewhat milky urine. Was not that much. The urine also had small, what seemed to be flakes in it. I have also noticed slight burning when I urinate. I seem to have the urge to urinate often. What could cause this?
