What Causes Cloudy Urine Despite Sufficient Water Intake?

I have been drinking a gallon of water per day, per my dr for three weeks now. I still never urinate clear and still have somewhat cloudy urine. Is this normal? I also have a sore on my tongue, the second one recently and have been getting random rashes.
