What Causes Constant Unfocusing Of Vision Along With Loss Of Memory?

I have constant unfucosing of my eyes and it happens so much on its own i have to shake my head like to shake my vision. I see everything like dreamstate and i have lack of motivation, cant feel real joy even wheni know i should i pretend. I cant focus so literally i do nothing cause i have no focus no clear view of the world no motivation and missing certain emotions. Please tell me what is happening im also loosing my memory at an alarming rate day to day and im only 25 years old
