What Causes Continuous Vaginal Bleeding?

my doctor has prescribed me norethisterone 5mg and i am unsure when i should start taking it as i am currently on clomid and i started taking it today the thing is i have irregular periods and currently i have just had a 4week continuous bleed and i have already come back on can you please help
