What Causes Cracked Swollen Skin With Puss And Blood On Hand?

my neighbor has both hands that look almost like poison ivy but it is not in season or in contact. they are cracked swollen with pus and blood. It is on the palms and fingers as well. the fingers look like if you stuck a pin hole yellow pus would come out and we have no idea what it could be.
