What Causes Crack,itchy And Dead Skin?

Hi , I am having a skin prob for last 5 years . extreme itching with dark black colour. the area has become black. black skins are having very tiny cracks and dead skins come out often by itching...tried several doctors...no result ...please tell me , where to contact in kolkata.
