What Causes Cracks On The Penis And Cloudy Semen?

iam male 64 years old iam a sugar patient
urine sugar fasting blood sugar 170 mgs/dl
post prandal blood sugar 266 mgs/dl
urine sugar(fasting) present (+)
urine sugar (pp) (++)
nowadays my sexual ank xity is low and my penis is lery loose always and not temper.
i find cracls at the front of my penis tip cover skin and my sperm was transperant earlier. but now i get white color sperm
kindly advise the reason.
