What Causes Delayed Periods Along With Vaginal Spotting?

I have been spotting and have not gotten a tea period...I have missed a pill or 2 and I had a cortisone shot in my back but have not gotten a full period....is there a good chance I am pregnant? And if so I have been taking pain meds would that be detrimental to my baby?
