What Causes Difference In Menstrual Flow Along With Irregular Periods?

I have always had abnormal periods ever since I started at age 12 im now 21 and I still get irregular periods sometimes I do. It get one other I get it lightly for weeks other times j get a very heavy flow for about 2/3 days with large blood clots I don not know why please help ?
