What Causes Difficulty For Ejaculation?

I am 49yrr, 5ft6inch, 63kgm. Good physic. I am diabetic and hypercholestrolaemia. Present sugar is 165 fasting. 195 PP. I m taking TAB GLYNASE MF 1BID ,ECOSPRIN AV 75 1OD. I do regular exercise, control diet. Now a days i feel a change on my penis. When it erect, it bend slightly left with a depression at mid of shaft left side. During sex it erect full strength but ejeculate with diffulty. what is the reason of change?
