What Causes Discharge Of Large Blood Clots Post Periods?

i recently had a normal menstural cycle but then two weeks later my period began again. i thought it was odd but then the next day when i went to change my tampon, my tampon was soaked in blood and i had what looked like a grape hanging from it that was also red. what is this?
