What Causes Discomfort In The Penis Hole?

My penis hole experiences discomfort when it is touched- I am uncomfortable walking around with pants on. But I experience no discomfort when there is nothing touching it. Also I do not see any irritation or feel any pain urinating or ejaculating. I was diagnosed with spermiotasis (I think that's the term for blood in semon) and just finished with the antiobiotics. Is this normal or a coincidence? Thank you
