What Causes Dry Penis Foreskin With Bleeding After Intercourse?

doc.i am 53 year old about 2 year ago my skin on the front of penis start to dry off and very time me and my wife has sexual performance.it would,t let my glans come out and than my skin start to cut the skin and start to bleed.it look like the skin is getting smaller please help me
