What Causes Elevated Blood Pressure Despite Taking Medication?

My uncle is 55 yrs old, non smoker for 3 yrs, recorded high blood pressure-taking medication, from his knees down they look red with a lacey pattern to it. He said he has bad knees from jumping down out of semi. Not hot or cold to the touch. Toes not cold or hot either.
