What Causes Elevated Sugar Levels Despite Medication?

My wife is on a pump. She has checked everything and still continues to be over 200. She has bolust more than usual. Still high she has checked insulin and connections and still won't go go down. Any ideas? She is wondering abou insulin intolerance. I can't find any good answers about how to diagnosis this.
