What Causes Elevated White Blood Count In Urine?

I have an elevated white blood count in my urine. its 25 and should be 4 per my doctor. I have a hienal hernia and take a reflux med for it, and most recently took an antibiotic for blood in my urine, hence my trip to the urologist to find the high blood count. what should be my next step?
