What Causes Elevation In Blood Pressure And Blood Sugar Levels?

I am 47 and i have not been taking care of myself. My diabetes is rated 310mg almost everyday. My High blood pressure is 152/102. I weigh 202. I have no idea how to take care of my blood sugar. I take medicine for BP Lisipril 20mg twice a day. I am getting worried. I worked 11-7 at night to morning. I should be sleeping now tired by to awake.
