What Causes Elevation In Creatinine Levels Along With Low Haemoglobin?

Hello, My dad is suffering from Kidney Issues. He has raised creatinine level by 4-5 pts. and Hemoglobin is also 5.3 grams only. Moreover he has urinary infection. He has history of water retention past 6 months for which he was admitted to Hospital and around 10 litres of water was taken out from his body. Since then he is under medication. Please suggest. He has High BP problem and Diabetes as well.
