What Causes Excruciating Pain In Lower Back Radiating To Upper Abdomen?

I have excruciating pain in my lower back and its spreading to my upper stomach and neck sometimes. It s like a lightening bolt when i inhale it worsens. Been to primary doctor and ER twice all i get is pain pills and muscle relaxants THESE THINGS DON T WORK. No tests or scans have been done. I don t know what to do
