What Causes Fainting,dizziness And Constant Headaches?

I'm 16 years old, i keep fainting every day once or twice a day i keep getting numb and cold legs then i can not feel them, i keep feeling sick now and again i keep getting constant headaches nearly every day, i always get spotty dots in my eyes and keep having dizzy spells
