What Causes Fatigue Along With Severe Sweating?

I have very bad fatigue whether I am active or not. I get shaky and sweat when doing anything. Even walking or preparing food in kitchen. Everything I do results in really bad exhaustion, shakes. Bad urine smell like a chemical or chlorine (don't even know how to describe it). Basically, I get these episodes at least 4-5 times per day.
