What Causes Feeling Of Choking On Phlegm?

I always feel like I am choking on my spit and I don't know if it is phlegm because when I cough really hard, phlegm comes out but this sort of feels different, it occurs most when I pay attention to it and when I go to sleep, why does this happen to me? What is it?
