What Causes Foggy Feeling While Waking Up In The Morning?

My mother in law wakes up every morning feeling foggy and many days feels out of sorts especially if she gets outside in the cold and her head and or ears get cold. She starts to feel better as the morning goes on and she can shake the cobb webs. She is 70 and very active even still works. It is not inner ear per her doctor. Any ideas?
