What Causes Frequent Nosebleeds With Clots And Headache?

my boyfriend (24 years old) keeps getting nosebleeds with blood clots. They used to be on just one side of his nose but last night was from both. He's also been getting more headaches than usual and is forgetful (sometimes even finds it hard to concentrate). Are these symptoms related and should he seek medical help asap or do you think it's from the cold weather (we live in Portland Oregon). He doesn't have insurance and is hesitant to seek medical help because he can't afford it but I'm worried about him. Thanks!
