What Causes Gradual Reduction In Functionality Of Heart?

My Dad is 72 years old. He suffered a hear attack in 2012. Post which his heart functionality came to 33%. He was undergoing treatment and the heart functionality improved to 47 % last year. This year his heart functionality is 32%. So is this a risk.
