What Causes Grayish White Tissue Like Vaginal Discharge?

my girl friend had a grayish white tissue discharge from her vagina and after she took a pregnancy test, it tested positive. we have been recently having unprotected sex but she was on birth control. does this mean she is pregnant? or was it possibly a miscarriage?
