What Causes Hard Lesion On Scab That Gets Irritated On Combing?

I have a lesion or scab, not sure what to call it on the back of my scalp. It's hard, not itchy, white with a red center that looks like a scab that may have bled. I've had this for months, not healing. Keeps getting irritated by combing. What should I do?
