What Causes Hard Lump On Forehead Along With Mushy Lump On Head Of An Infant?

My 1yr old fell onto the concrete on his first birthday , he was in a carseat & went face first , he now has a big bump on his forhead going up to the top of his head , The bump on his forehead is hard but the bump ontop of his head is mushy.. Should i take him to the ER or take him to his doctors ?
