What Causes Hardness Of The Cheek And Bleeding With Clots Post Injury?

My mother fell and hit her cheek bone about 2 weeks ago and she is still passing clots of blood and her cheek is still very hard at the impact of the fall. Can you advice me the reason for the blood discharge and the still hardness of her cheek?
