What Causes Headaches, Earache And Dizzy Spells?

I have severe headaches, earache and dizzy spells that last for 3-4 days. During these times I cough up phem at night (this starts when I lie down. The it all goes away for a month or two. Had this for 2 years now. Had CT scan for sinus problems but does not appear to be that. 48 smoker, slim build, very busy lifestyle can you help?
