What Causes Headaches, Fever And Lack Of Appetite In A Child?

I have a five year old. He is complaining with a headache, has a low grade fever no appetite or energy. I took him to the doctor and said he tested negative for flu and strep. They said one of his ears was really red and gave him a antibiotic. In my past experience I have never seen a ear infection make a kid this sick.
