What Causes Heart Palpitations And Arm Pain While Standing Up?

When I stand up I have heart palpitations and arm pain. When I am laying down my heart rate is bout 80 to 90 when I stand up its around 125 to 130. I have all heart test done except heart cath, but I have had a ct aniogram and they all say heart is fine. What is it?
