What Causes Heavy Bleeding With Clots During Menopause?

I am 52 years old with a history of heavy bleeding and fibroids. I am perimenopausal. I am taking no medication at the present time. My body hasn't had a normal menstruation for more than 20 years, but recently, my period has been nonexistent. However, today, the flow is continuous and very clotty (clots the size of half my palm). Is there any treatment I can do at home to ease the bleeding? I am not suffering any cramps, just messy bleeding.
