What Causes Heavy, Long Periods With Clots Of Blood In Urine?

I am a 43 y/ o female who has been having irregular periods for years, and sometimes when I get my period I bleed to the point to where I have to wear bladder control pads. I also have anemia. This month my period has been on for ten days. It is not coming down too bad but sometimes when I urinate, I see clots in the toilet or if I use the bathroom and wipe I see blood. I dont have any pain. I have a history of the following hypertension, ckd, thyroid (hypothyroidism), anemia, and sciatica, I also had a tubialigation in 2008 and gastric bypass in 2013. I am puzzled about whats going on with me.
