What Causes Heavy Periods With Clots?

I m 43 and My periods have started to become very heavy with large clots. This month it lasted for 10+ days and was so heavy the entire time that I used ultra tampons along with overnight pads and had to change both about every 2 hours. I also experienced clots, some as large as 4 in diameter. What could be causing this.
