What Causes High Cortisol Level When Suffering From Congenital Adrenal Hyperplasia?

I have almost zero aldosterone ( 1.0) lower end acth (11.2) and high cortisol (28.4). I was diagnosed with congenital adrenal hyperplasia 34 years ago, but how does high cortisol fit into the picture with these kinds of Aldo and acth numbers? When I am on dexamethasone, my acth and cortisol are in very normal ranges, but my current endo doesn t think I need it, or even Florinef! So I am going without treatment, and faint quite frequently. I am confused!
