What Causes High Fever And Pain In The Ear?

hi,my sister's son is almost 4 years old,because of high fever and pain in his ear ,he is admitted 4 days in hospital,and in his blood test the rbc is high,before it was 270 and today it was 195..is it dangerous?may i know all the issues about it and please explain me...thanks
