What Causes Hoarseness Like Laryngitis?

Hi our friend has hoarseness like Laryngitis he has no pain,he has had it for almost 8 weeks he has a chronic cough.we keep thinking he is going to choke.he is also very weak and just wants to sleep his wife has tried to get him to go see a doctor he will not go He does not smoke.any idea what this could be.Thanks
