What Causes Hole In Eardrum Along With Mild Headache And Nausea?

hello I have a broken speaker filling in my right ear. I have a mild headache that comes and goes. My own voice or even walking causes discomfort in my ear. I have some mild nausea. I have tried taking over the counter medicine for vertigo and motion sickness as my MD. has recommended.
