What Causes Hole In The Ear Drum With Discharge Of Pus And Water?

I'm 20 year old girl.My problem is i'm having hole in my right eardrum.
I don't have much pain but pus & water comes from my ear. ENT Dr adviced me
to undergo surgery. He told if i didn't takeup the surgery now i wil loss my hearing capacity in future.Is it true?Is there any treatment available for my problem other than surgery? Because i'm affraid of surgery. And i also want to know the cost & after effects of the surgery?
