What Causes Increase In Monocyte Count?

My monocyte count has gone up steadily for the last 3 years. It is now 23. My blood test detected anticardiolipon antibodies. I have factor v leiden and am on xaralto after being on coumadin for five year., I did have blood clots in lungs 5 years ago after being on hormone drug for breast ca. I am just now going to hematologist and he is indicating doing a bone marrow test. Should I get a second opinion
