What Causes Intermenstrual Bleeding?

I have been having spotting between periods. I am on the pill. The discharge is a brownish reddish color. There is some red mixed in, but this is day #4 of this, so now it is mostly brown. I also am having some cramps. I am sexually active. Am I having a miscarriage?
