What Causes Itching And Swelling On Nipple Of Breast?

i have a very itchy niple and it looks swollen sometimes it feels like a shooting pain. I am breast feeding and this pain started a few weeks ago. my doctor prescribed canesten cream and no change and now i have been on antibiotics for three days and im still in pain. They think its thrush or infected. can it be anything else? please help
