What Causes Itching On The Distended Umbilical Hernia?

Hello! My husband has had an umbilical hernia for the past 8 to 9 years. Currently it's distended and he's been experiencing an itching sensation...under his skin. Is this normal? My husband is 39 years old, is at least 280 lbs has high blood pressure and diabetes...all untreated due to no insurance.
