What Causes Itching On The Scalp With Severe Hair Loss?

i am suffering from hair loss from last 4 year. i think problem is with sculp which becaome more itchy during summer season. during hair fall rate is more and scalp leave crust about 1cm. i have shown it to doctor he has given me ketokatazone which does not cure it. is it fungul infection? whet is remady?
