What Causes Itching On Toes?

Hi,
I've been having this "stinging/itchy sensation of my left and/or right big toe only. The first time was in June 2013 & then September 2014. This started again 01/10/2015, Saturday night before 11p.p.m. Weird, I don't understand it. I massage, and rub m big toe but to no avail. This is an itch I just can't seem to scratch. The sensation is from the inside not the outside of the toe. I don't understand it and my doctor is baffled since there is no pain, no swelling, no redness, no cracking, no dryness. I've tried every over the counter creams, foot & fungi creams recommended by my pharmacist and home remedies including vick's vapor rub. I've read & re-read & still can't find an answer that involves the stinging & itching only without the pain.
I am a diabetic with nephropthy, arthritis, gout. I take uloric for my gout. I don't have gout glareups since I started using Uloric over three years now. I'm exhausted. When I'm having one of these attacks, it will wake me a night & I sit on the edge of the bed just massaging & rubbing & sometimes crying because I can't get to the itch & I can't sleep until it calms down which can take from 2 0 minutes to an hour. It will then be silent for 3-5 hours before it slowly builds up to that itch again. Any help you can provide will be greatly appreciated.
