What Causes Itchy, Black Scar On Hands With Pain If Exposed To Sun?

I have a skin problem in my both hands, there's a itching black scar in both my hands, i cannot go out to sun or expose my skin then it will be red & painful, i dnt know what is this exactly please explain, also it looks so ugly, i cannot wear short sleeves,please help.
