What Causes Itchy Legs,loss Of Appetite And Extreme Fatigue?

My son aged15. He had fatigue, loss of appetite, itching of his legs. His skin is yellow colour. He also had bilirubin show up in his blood test. He had a scan done today on his liver. Liver ok but slightly enlarged spleen. Would you have an idea what it could be?
