What Causes Itchy Raised Spots On Hands, Legs And Back?

How can I tell if the rash (little raised places which are sometimes dry and sometimes not but which itch as if they were poison ivy) breaking out on my hands, legs, and other places on my back is eczema? What is eczema, what does it look like, and how does it affect the body?
