What Causes Itchy Rash Behind The Knees?

for 4 years I have had a constant rash behind the knees that itches and I the skin peels away on my fingertips to where I have multiple layers, with cracks. Now I have a rash starting on my ankle. I've tried various creams - nothing is working. don't have any other health issues.
